Discretionary Point Scheme form (Office document, 47kB)

For use in making a case for rewarding and recognising the exceptional personal contributions of staff employed on the Senior Lecturer/Reader grade L, and on Grades K/L for senior support staff
Flexible Working Flexible working requests can be made via MyERP.

Maternity leave

  For use in making a case for a merit payment to recognise those who have successfully delivered a one-off task or project that would not normally form part of their role, or have contributed significantly in some other way that would not normally be expected of their role, and for which there will not be a continuing requirement.

  • Paternity/partner leave application form - to be completed by eligible employees who wish to take up to 2 weeks' paid leave following the birth or adoption of a child. Further leave for the partner can be taken under the shared parental leave policy.

Shared parental leave

Sickness absence Sickness absence notification forms, to be completed by staff on return from sickness absence, are held in MyERP.
Development Review for staff
  1. Frontline staff with limited or no access to computers.
    Staff in this category can complete a paper review form with their reviewing manager. The form is available on the Staff Development website.
  2. Any staff member with more than one role at the University.
    If you have multiple job roles at the University and need to carry out more than one Development Review for the period, please contact sd-course@bristol.ac.uk

Study Leave Form for Professional Services Staff (Office document, 52kB) To be completed by an individual who wishes to apply for time off for training in accordance with the Study Leave policy (support staff) (PDF, 156kB)
Unpaid Leave Application Form (Office document, 52kB) To be completed by individuals who wish to request a period of ad-hoc unpaid leave not covered by bereavement, emergency, dependant and carers leave, unpaid parental leave or Study Leave (Professional Services or Academic).
