1984 to 1988
Incident IHD - all IHD events between Phases I and II
- Death: deaths coded to ICD 410-414 (9th revision)
- Myocardial infarction: severe chest pain on the LSHTM questionaire, plus HAA listed events (details in hospital case-notes checked against WHO criteria).
- ECG Myocardial infarction: no Q/QS wave (1-1, 1-2, 1-3) at base line, but major or moderate Q/QS waves (1-1-1 through 1-2-5 plus 1-2-7) on follow-up
