As part of the study we are sending our participants and their family questionnaires at regular time points to gather outcome data. Please find copies of all our questionnaires below:

Participants are sent a questionnaire at baseline, 6 months and 12 months. We have three different versions of the questionnaire:

One for participants under 12 Questionnaire E (Office document, 352kB)

One for participants over 12 Questionnaire F (Office document, 377kB)

A reduced questionnaire, for participants who do not want to complete the full version Questionnaire D (Office document, 296kB)

 

Participant's relatives are also sent out a questionnaire at baseline, 6 months and 12 months.
