Collaborator Checklist

In those instances where a FMD staff member is participating in a project as a collaborator and where research ethics approval has been granted by another institution, the FMDCE would like to view the following before research commences:

  1. A copy of the application for ethical approval at the relevant Institution, including a full description of the study;
  2. A copy of the approval letter/email;
  3. Information about the composition and authority of the awarding body (this can be a link to the relevant website); and
  4. Details of the contribution to be made by the UoB collaborator

At its discretion the FMDCE can also request other documents it considers relevant to the study.

Where UoB researchers are engaged as collaborators on a project where the Principal Investigator is based in another institute, the senior UoB researcher is deemed to be the responsible person acting for the UoB. Where the role of UoB researchers in the project is to recruit subjects and/or to collect samples or data directly from subjects, the FMDCE requires the full checklist to be submitted. Where the role of UoB researchers is confined to processing of samples or data analysis and no contact occurs with subjects, only Item 2 from the above list is required.
