Health & Safety questionnaire

Your welfare and wellbeing means everything to us. That’s why, once your placement’s started, you must fill out a short Health & Safety questionnaire. It makes sure that your host company has thought about your health and safety needs, and that you’ve had a proper induction into your new workplace.  The questionnaire for students abroad in 2011/12 is now open and you must take it in order to receive your Erasmus grant. Online feedback survey and employer letter

If you are doing an Erasmus work placement in 2011/12 (this includes British Council teaching assistantships) we need you to fill out and submit an online survey within one month of your placement finishing. If you don’t, it could result in the SWAP Office claiming back all your Erasmus Grant.

We’ll also need an employer letter (on official headed paper, with their official stamp) saying that you’ve completed your work placement, and confirming the exact start and end dates. Again, you should get this to us within one month of your placement finishing. We need to have your online survey results and your employer’s letter, before we can authorise your second grant payment. (You need to do both the survey and letter for each work placement.)

During your placement

If anything changes with your placement, if you withdraw from it, if it’s shortened or extended, you need to let the SWAP Team know as quickly as possible. And while there isn't any paperwork to send us at the SWAP Office keep an eye on what the the School of Modern Languages may need from you.
